Wikipedia has lists of deaths and major accidents. The WDW list is fairly long but not when you consider park attendance totals 50 million plus per year. Many of them seem to be from pre-existing medical conditions too, rather than operator error/mechanical failure.

1998? My c.2001 Kodak Easyshare camera had a massive two megapixels with no optical zoom and still cost $600+. There was no zooming with quality shots.
